On this day, February 1, 2003, Space Shuttle Columbia disintegrated during re-entry into Earth's atmosphere after completing its STS-107 science mission. The cause traced back to launch, when a piece of foam insulation struck the shuttle's left wing.
The impact created damage that allowed superheated air to enter the wing during re-entry. At speeds over Mach 18, the structure failed, and the vehicle broke apart.
